This tool can be used to try and improve the situation for your loved one. Sharing the log of unacceptable observances with the management team of the Nursing Home may help to rectify the situation as they see you as someone who pays attention and DOCUMENTS what is observed at the facility. If communication doesn't result in improved/acceptable care, your documentation can be useful within the context of a legal consultation, and/or to report elder abuse to appropriate state/federal agencies.
